English Translation
It was narrated from Ibn 'Umar (رضي الله عنهما) — via Muhammad ibn Ja'far, from Shu'ba, from Khalid, from 'Abdullah ibn al-Harith — that he instructed a man to say upon taking to his bed: 'Allahumma innaka khalaqta nafsi wa anta tawaffaha, laka mamatuha wa mahyaha — in ahyaytaha fahfadha, wa in amattaha faghfir laha. Allahumma as'aluka al-'afiya [O Allah, You created my soul and You take it away; to You belongs its death and its life — if You grant it life, protect it; and if You cause it to die, forgive it. O Allah, I ask You for wellbeing].' A man said to him: 'Did you hear this from 'Umar?' He said: 'From one better than 'Umar (رضي الله عنه) — from the Messenger of Allah ﷺ.'
